The CARI Guidelines – Caring for Australians with Renal Impairment Evaluation of Renal Function

چکیده
• There is no convincing evidence that serum cystatin C measurement offers any clinical advantage over estimated glomerular filtration rate (eGFR) (Level III evidence, small-to-medium observational cohort studies, surrogate outcomes, weak effect) except that serum cystatin C appears to be superior to both serum creatinine and eGFR as a predictor of cardiovascular events in the eldely (age ≥ 65 years). (Level III evidence, large population cohort study, clinically important outcomes, strong effect)

Proteinuria in a single urine specimen is relatively common in children, with a reported prevalence between 1%-10% (Hogg et al 2000; K/DOQI guidelines, Guideline1, Part 4). However, persistent proteinuria is much less common, with one American study of nearly 9000 school children reporting proteinuria in 1 out of 4 samples in 10.7% of children, but proteinuria in all 4 samples in only 0.1% (Veh...

In non-diabetic CKD patients, the hypertriglyceridaemia can be reduced by both increasing the dietary polyunsaturated: saturated fat ratio and by reducing the carbohydrate content of the diet. Dietary modification under the supervision of a suitably qualified dietician may be helpful for managing the hypertriglyceridaemia. Patients with other coronary risk factors (i.e. smoking, hypertension, o...

a. Patients receiving automated peritoneal dialysis (APD) treatment report significantly more time for work, family and social activities (Level II evidence; single good quality RCT, clinically relevant outcome, strong precise effect). APD should be prescribed in preference to continuous ambulatory peritoneal dialysis (CAPD) for patients in whom minimising the time spent performing peritoneal d...

• Diabetes and Aboriginal and Torres Strait Islanders. Initial testing for albuminuria is preferred to proteinuria as it allows detection of early nephropathy. In patients with established nephropathy there is no evidence that either test is superior. Recommendation: Initial test – urine ACR. Subsequent testing: initial test negative or microalbuminuria – repeat urine ACR initial test macroalbu...
متن کاملThe CARI Guidelines – Caring for Australians with Renal Impairment Urine Protein as Diagnostic Test (October 2004) 2. Performance characteristics of tests used in the initial evaluation of patients at risk of renal disease
End-stage renal disease (ESRD) develops in about 1500 people each year in Australia, of whom about 1000 are over 50 years of age (a risk of about 200 per million per year). Diabetes, hypertension, impaired renal function and proteinuria are likely independent risk factors for the development of ESRD. About 5% of the general population have proteinuria and these individuals are approximately 15 ...
